Video Summary
ACPM hosted a webinar on preventing falls in older adults, introducing the new “Take 10” follow-up tool built around the Stay Independent/STEADI 12 questionnaire. Dr. Ian Ara explained that falls should be viewed as a chronic condition requiring longitudinal, team-based care rather than a one-time screen-and-intervene event. He outlined the burden of falls in older adults, then walked through the tool’s workflow: initial screening with Take 5, followed by a more detailed assessment for patients at increased risk, and then targeted interventions based on domains such as mobility, medications, vision, footwear, environment, and fear of falling.

Three case examples showed how the tool can guide care for lower-risk, higher-risk, and community-program-focused patients. The discussion emphasized reassessment over time, coordination across primary care, physical therapy, pharmacy, and community programs, and adapting the tool to different practice settings, including rural clinics. Audience questions addressed medication tools, CPT codes, reassessment timing, and how Take 5 differs from Take 10. ACPM noted that additional resources, including coding guidance, will be added to the webinar webpage.
