Preventing Falls in Older Adults: Operationalizing the Stopping Elderly Accidents, Deaths, and Injuries (STEADI-12) Action Tool
Description
This webinar titled, Preventing Falls in Older Adults: Operationalizing the Stopping Elderly Accidents, Deaths, and Injuries (STEADI-12) Action Algorithm, introduces the STEADI-12 Action Toolkit, a practical subsequent-visit tool building on ACPM’s Take Five for Fall Prevention initiative. The webinar demonstrates how healthcare teams can translate STEADI-12 responses into prioritized, domain-specific actions, apply the decision-tree algorithm, and integrate a team-based, longitudinal “Safe Mobility” approach into clinical workflows.
Outcome Objectives
  • Apply the STEADI-12 decision-tree algorithm and supporting table for team-based clinical decision-making
  • Explain the purpose and design of the STEADI-12 Action Toolkit as a practical subsequent-visit tool
  • Translate STEADI-12 responses into prioritized, domain-specific actions
