Diabetes Prevention Webinar Series: Independent Practice Associations
About the Series
Through a partnership with the CDC Division of Diabetes Translation, ACPM funded six health care organizations/practices in the fall of 2017 to develop and implement a practice protocol for screening, testing, and referring patients with prediabetes to a CDC-recognized diabetes prevention program, either through the EHR or by using another non-electronic approach.

To share information about the experiences and lessons learned through this demonstration project, ACPM will hosted a series of three free webinars this fall featuring the work of the project grantees. Webinar participants will have the unique opportunity to learn how to evaluate, develop, and operationalize screening and referral models that fight the diabetes epidemic.

The six grantees are organizations or practices from three different provider settings: integrated practice associations, integrated delivery systems, and federally qualified health centers. Each will share the results of their protocol models.
Learning Objectives
Upon completion, participants should be able to:
  • Explain key components the National Diabetes Prevention Program (National DPP) 
  • Identify processes for screening, testing, and referring patients with prediabetes to a CDC-recognized diabetes prevention program 
  • Evaluate screening and referral models and ways to develop, and operationalize these models

About the National Diabetes Prevention Program (National DPP)
ACPM partners with the Division of Diabetes Translation at CDC to increase physician awareness, screening and referral to the National Diabetes Prevention Program (National DPP).

The CDC-led National DPP is a partnership of public/private organizations working collectively to establish, spread, and sustain an evidence-based lifestyle change program for people with prediabetes to prevent or delay onset of type 2 diabetes. The partners work to make it easier for people with prediabetes to participate in evidence-based, affordable, and high-quality lifestyle change programs to reduce their risk of type 2 diabetes and improve their overall health.

The goal of our work with CDC is to increase physicians’/health care professionals’ awareness of prediabetes as a serious health condition and to increase the number of physicians/health care professionals taking action to screen, test, and refer patients with prediabetes to CDC-recognized diabetes prevention programs (programs with pending or full recognition).

ACPM partners with CDC and other organizations to develop, coordinate, implement and manage educational trainings, as well as, gather stakeholder input from experts in the field. This includes:
  • Providing educational and experiential learning opportunities for physicians/health professionals through existing professional networks, meetings/conferences, web-based forums, online tools, and publications (journals, e-newsletters, etc.) 
  • Conducting Physician peer-to-peer mentoring and outreach 
  • Develop physician "champions" to serve as spokespersons and advocates for increasing prediabetes screening, testing, and referral 
  • Demonstration projects to evaluate innovative approaches to engaging and activating practitioners in targeted medical specialty groups or specific geographic areas of need
Demonstration Projects
ACPM has funded 9 health care organizations/practices to develop and implement a protocol for screening, testing, and referring patients with prediabetes to a CDC-recognized diabetes prevention program, either through the EHR or by using another non-electronic approach.
