Get access to our Innovative Systems Approach to Achieve Health Equity in Diabetes Prevention lecture and evaluation, in collaboration with our PM22 collection.

In this lecture, you’ll hear from eight key speakers on the importance of diabetes prevention for communities, including the Hispanic and black population. You’ll learn about the Diabetes Prevention Program, a cooperation between the American college of preventative medicine and the American medical association (AMA) and the black women’s health imperative (BWHI) that focuses on addressing and assessing social determinants of health.

Upon completion of this lecture and evaluation, you should be able to accomplish the following:

• Articulate two evidence-based strategies to screen and test disproportionately affected populations and refer patients with prediabetes to the National Diabetes Prevention Program (National DPP).
• List at least two high touch coach tactics/tools to enroll and engage participants within the National DPP.
• Describe at least one promising practice to assess and address social needs barriers of patients with prediabetes who are referred to the year-long National DPP lifestyle change program.
