This tool is designed to assess a diplomate’s integration of preventive practices and is used for both adult and pediatric patient populations. To complete the CPE II – Clinical Systems Review, diplomates need to conduct a comprehensive analysis of various aspects of their clinical operations, such as information management, patient activation, access and communication with patients, safety and efficacy, consultation and referral processes, practice team dynamics, and improvement processes.<br /><br />Key focus areas include:<br /><br />1. <strong>Information Management</strong>: Diplomates are required to report on their use of electronic medical record (EMR) systems, evaluate the functionality of various elements, templates, and reminders within their medical record system, and assess features related to patient documentation and preventive services.<br /><br />2. <strong>Patient Activation</strong>: This section evaluates the practice’s provision of written information, behavior change practices, tobacco cessation, nutrition, and physical activity counseling services, and methods for teaching self-care skills.<br /><br />3. <strong>Access and Communication</strong>: The review includes evaluating the practice’s appointment scheduling features and the effectiveness of communication between patients and providers.<br /><br />4. <strong>Safety and Efficacy</strong>: Diplomates must assess practices aimed at reducing the risks of medication and diagnostic errors, delays in receiving care, and increasing overall practice efficiency.<br /><br />5. <strong>Consultation and Referral</strong>: The tool gathers information on the size and type of practice, the frequency and nature of patient referrals to consultants, and the provision of consultative care by the practice.<br /><br />6. <strong>Practice Team</strong>: Assessment of the practice team’s structure and cooperation is required.<br /><br />7. <strong>Improvement Process</strong>: Diplomates must describe their practice's efforts to undertake systematic improvements.<br /><br />After completing the CPE II, diplomates should retain all forms and submit the evaluation and supporting documentation to the American College of Preventive Medicine as part of the DPAP cycle.
