Responses are categorized as Yes, No, Unsure, or N/A based on appropriateness related to the patient’s age, gender, or risk status.<br /><br />**Part E: Intervention**<br />- Confirm if the patient received counseling for specified clinical situations, chemoprevention interventions (like medications or vaccinations), and note any conditions limiting the patient's ability to engage in age-appropriate activities.<br /><br />The process must be repeated for a total of ten pediatric patients. After reviewing all charts, the completed evaluations and supporting documentation will be submitted to the American College of Preventive Medicine as part of the Diplomate Practice Assessment Plan (DPAP) cycle.
