This section gauges how consistently these safety measures are applied.<br /><br />5. <strong>Consultation and Referral:</strong><br /> - Verify the size and type of practice.<br /> - Assess referral practices to consultants and conversely, the provision of consultative services by the practice.<br /><br />6. <strong>The Practice Team:</strong><br /> - Review the composition and functionality of the practice team.<br /><br />7. <strong>Improvement Process:</strong><br /> - Describe methods for practice improvement, with responses indicating the consistency of these practices.<br /><br />Completed evaluations, along with supporting documents, should be retained and sent to the American College of Preventive Medicine, concluding the Diplomate Practice Assessment Plan (DPAP) cycle. This thorough assessment aims to ensure high standards in preventive medical practices within various clinical systems.
