The 360-Degree Feedback Evaluation is aimed at providing comprehensive feedback for diplomates of the American College of Preventive Medicine to aid in professional improvement. The evaluation process involves self-assessment and feedback from multiple evaluators in different professional roles: a supervisor, two peers, and one reportee.<br /><br /><strong>Instructions for Diplomates:</strong><br />1. Select 10 competencies to be evaluated from the 360-Degree Feedback Evaluation tool.<br />2. Perform a self-assessment on these 10 competencies. Identify yourself and circle “Self” under “Evaluator.”<br />3. Each competency comprises specific performance indicators rated on a scale from 1 to 4:<br /> - 1: Rarely demonstrate the skill (less than 25%)<br /> - 2: Sometimes (25-50%)<br /> - 3: In most cases (51-75%)<br /> - 4: Nearly all cases (more than 75%)<br />4. Distribute copies of the evaluation tool and instructions to the evaluators mentioned. Include the selected competencies.<br />5. Collect completed evaluations from the evaluators.<br /><br /><strong>Instructions for Evaluators:</strong><br />1. Identify the physician (diplomate) and circle the term that best describes the relationship (Supervisor, Peer, Reportee).<br />2. Evaluate the diplomate on the 10 selected competencies marked by an “X.”<br />3. Rate each competency's performance indicators on a 1-4 scale similarly to the self-assessment scale.<br />4. If unable to assess a performance indicator, mark "Unable to Assess."<br />5. Provide comments on each competency in the comments section at the end.<br />6. Return the completed evaluation to the diplomate.<br /><br />The feedback gathered through this method aims to help diplomates design and evaluate practice improvement or professional development plans. Only one competency will be selected for re-evaluation during the re-assessment phase. The collected feedback and evaluations will be sent to the American College of Preventive Medicine as part of the Diplomate Practice Assessment Plan (DPAP) cycle.
