This document provides comprehensive guidelines for completing a Report Card Evaluation for diplomates of the American College of Preventive Medicine. The process involves selecting ten competencies for evaluation out of a total of thirty-three listed. Diplomates are required to write their complete name on each page before initiating the self-evaluation.<br /><br />For each selected competency, diplomates assess their performance against specific indicators and document their ratings and example behaviors in the designated columns of the Report Card Evaluation tool. Following the self-evaluation, the completed evaluation form must be handed over to a supervisor or knowledgeable colleague for an independent assessment.<br /><br />Evaluators are instructed to rate the diplomate's level of demonstration of each competency based on their observations, separate from the diplomate's self-assessment. They are to use the same rating scale and document their assessments and observed behaviors accordingly. Evaluators then return the completed assessment to the diplomate.<br /><br />Diplomates must keep the completed evaluation as it forms an essential part of their Diplomate Practice Assessment Plan (DPAP) and must be filed with the American College of Preventive Medicine upon completion of the DPAP cycle.<br /><br />For re-evaluation, only one competency will be assessed instead of ten. The process aims to provide meaningful feedback to develop and evaluate practice improvement or professional development plans efficiently.
