The review is broken down into sections:<br /><br /><strong>Part A: Record Identifier Information</strong><br />- Document review date, create a patient identifier, note the most recent visit date, record patient gender, and age.<br /><br /><strong>Part B: History</strong><br />- Confirm initial and interval history documentation during the patient’s visits.<br /><br /><strong>Part C: Objective Findings</strong><br />- Assess documentation of physical findings such as head circumference, height/length, weight, Body Mass Index (BMI), and blood pressure.<br /><br /><strong>Part D: Screening</strong><br />- Evaluate documentation of newborn, childhood, and adolescent screenings for various health conditions following the United States Preventive Services Task Force (USPSTF) recommendations. Responses are categorized as Yes, No, Unsure, or N/A based on appropriateness related to the patient’s age, gender, or risk status.<br /><br /><strong>Part E: Intervention</strong><br />- Confirm if the patient received counseling for specified clinical situations, chemoprevention interventions (like medications or vaccinations), and note any conditions limiting the patient's ability to engage in age-appropriate activities.<br /><br />The process must be repeated for a total of ten pediatric patients. After reviewing all charts, the completed evaluations and supporting documentation will be submitted to the American College of Preventive Medicine as part of the Diplomate Practice Assessment Plan (DPAP) cycle.
